:Proview:

The Proview™ home eye pressure monitor is as accurate as Goldmann tonometry, the in-office gold standard for measuring Intra-Ocular Pressure (I.O.P.). Proview™ requires no anesthetic and eliminates the risk of corneal injury by measuring through the eyelid.

Inside this device is a small spring. As the device is used, the spring is depressed, which in turn pushes a small arrow up a scale printed on the side of the device. This scale runs from 8 to 40 in increments 2 mm apart. IOP is measured in millimeters of mercury ( mm Hg ).

The Proview™ works by stimulating a pressure phosphene in the retina, which is experienced as a visual light sensation. If you can see a phosphene when you press on your eye through the eyelid with your fingertip, you can probably learn to use the Proview™. Try looking way down and to the side (right with the right eye, or left when testing the left eye). Then lightly press on the eyeball through the upper eyelid, near the bridge of the nose. Do you see a light sensation somewhere opposite to the position you are pressing on? This is a phosphene.

Proview™ is great when it works. It does not work for those with a lot of peripheral vision loss, such as advanced cases of glaucoma, nor for those with very poor motor coordination, but for most people with early to moderate vision changes it gives a real boost to IOP data gathering, which can really help you and your doctor.

You can test at any time of day to easily monitor diurnal rhythms, which are so important in glaucoma and so hard and expensive to measure in a 9-5 office routine. You can also watch for and begin to track down any fluctuating reactions, as from allergies, medications, excess fluid intake, etc.

Some of the benefits:

  • Makes contact with the eyelid only: no anesthetic required.
  • Allows for insights into patient-specific, diurnal variations.
  • Tracks the impact of drug therapies and may help with adjustments to dosage and/or regimens.
  • More active patient involvement may enhance compliance.

Wills Eye Hospital glaucoma specialist Dr. Marlene Moster suggests possibilities for improved care with the Proview™ device:

  • A more accurate picture of a patient’s IOP (Since IOP tends to vary throughout the day and night, and the pressure the doctor measures in the office, maybe every month or even every week, cannot reflect this variation, the Proview could permit a more accurate picture without the patient having to spend all day in the office, having the pressure checked at regular intervals and still miss most of the hours of the day, including the early morning that is often the most critical.)
  • Reduction of patients’ anxiety about their glaucoma (an important factor in healing glaucoma since stress is one of its major contributors)
  • A convenient reminder for patients to use their glaucoma medications consistently.

The Glaucoma Research Center’s 10-month, randomized, prospective study of the device conducted by Dr. Moster found that 92% of the 100 patients in the treatment group described the device as easy to use and 74% learned the technique within 4 days. The key conclusions of the study are that:

  • The correlation between Goldmann and Proview™ measures is best in the normal range of IOPs of 10 and 20 mm Hg.
  • The treatment group reported significantly less anxiety after 1 month’s use.
  • A majority of patients reported continuing compliance with their ocular medications.

The results show that the Proview™ readings should initially be calibrated with the Goldmann readings for each patient to be sure that accurate conclusions are reached by the doctor interpreting the home test data. Dr. Moster concludes, “The concept of the Proview is an important advance, potentially giving patients a significant opportunity to participate in their own care.”

Proview™ is available for a donation in the range of $75.00 to $114.95 from the Remission Foundation.
